  • A stay-in, post-exacerbation palliative rehabilitation program reduces re-admission, hospitalisation length-of-stay and ED visits in patients with chronic lung diseases.
  • Frail older persons with significant deconditioning demonstrates greatest functional improvement with such a program.
  • Early integration of palliative care with functional rehabilitation leads to increased identification and treatment of dyspnea, anxiety and depression.
